Generally elderly people raise a question-“how long will I live” ? The right question which one should raise is – “ if I am going to live such a long time, how can I be happy in the process “?

The common worries of elderly are- “I am losing my health”, “ I am losing the ability to take care of myself”. In late life person’s health declines, he loses youthful looks, feels functional loss and becomes physically vulnerable. There arises a gap between what he aspires to do and what he is able to do.

There are specific consequences of age related declines ,such as, memory impairment ( it can create a fear of loss of meaning as you get older), physical disability etc. Age related decline is different from decline due to disease. Age related decline occurs gradually in stages such as, dy-function, disability, dependency and death. The pathway through these four stages of ageing is unavoidable. The decline will affect you and influence your life satisfaction and well-being. This process of decline is termed as frailty which is not connected to illness and disease. Being frail means you are declining, it does not mean that you are weak or unhealthy.

When you are old society expects you to act according to your age. They have negative attitude about old age and aged. The negative attitude towards elderly ( ageism) will affect elderly what he does and how he feels about his ageing experiences.

SELF-CENTEREDNESS: BARRIER TO POSITIVE AGEING

Self-centeredness or self-absorption in old age is a complex phenomenon. It is a maladaptive quality related to coping and subjective well being in the last phase of life. The person having self-absorption style of life fails to develop other focused qualities, (which are the signs of wisdom) in old age. Such persons have difficult time in experiencing positive ageing.

Self-centeredness has many forms. Self-absorbed person focuses on him or herself and give priority to the meeting of personal needs, desires, appetites and preferences. They do not give importance to the needs of others (even to their family members or care givers). Self-focus may be of two forms-(a) I am special and (b) something is wrong with me. These two forms interact with the ageing process. The elderly thinks-“I am special because I am old” and should be cared first. When he says that “something is wrong with me”, he is of the view that due to my vulnerability to disease and disability I should be given priority.

WORRY: BARRIER TO POSITIVE AGEING`

Worry is a mental pre-occupation. It is always not bad. Sometimes it works as a motivation or may help in making a potentially reasonable strategy to deal with stressful life situation. In other words, it helps an individual in the preparation for dealing with a future challenge or a threat. In this sense worry can be adaptive. It is true that an elderly may have real life event about which to worry. For example, the cause of worry may be one’s poor health, worry about the sickness of family member or death of a close relative, financial insecurity, changes in one’s own functional independence and dying. When an elderly adopts some strategy to overcome that situation then his worry is adaptive.

On the other hand, worry may consume your time and energy if it goes on excessively on the issues or things that are not worth worrying about. Such a situation is a negative psychological barrier effecting positive ageing. Excess worry over trivial issues will have depressive effects on your mental and physical health.

Person who has developed worrying life style become over sensitive about smallest things. The person may develop physical illness or physical aches and pains which may result in narrowing focus on life challenges even when it is not possible to do much about them.

MORAL OBLIGATION TO CARE FOR NEGLECTED SENIOR CITIZENS

The rapid ageing of the world population* has brought about visible changes in the familial, social and economic spheres of human life. The changes have affected areas like the intergenerational equilibrium of population, long term care, gender ageing, and socio-economic condition of elderly. As people live longer, specially the old-old and oldest old, have longer dependent life too. The situation for the poor elderly and also of those who are living alone is more difficult because they have to depend on others for their security (economic, social) and health care. The advancing age brings along with it health concerns like frailty, morbidity and vulnerability to abuse, exploitation and neglect.

Vulnerability implies that the personal integrity of the elderly can be harmed or violated because he/she is too weak to protect him/her self. It is precisely because the elderly is vulnerable to harm or violation that we are required to work towards the protection of his integrity.  The principle of vulnerability poses before us a normative value. What is central to vulnerability of elderly is the loss of power due to economic and social insecurity, chronic illness or inability to get long term formal (directly from the government) or informal (from community or family) care. It is the fear that one is no more master of one’s own life. Every human being has a three fold urge, namely, to live, live well and live better.1

Skin Pigmentation

Pigmentation disorder in human skin occurs as a result of the body skin producing either too much or too little melanin (A pigment that is ubiquitous in nature, being found in most organisms.). This skin disorder creates a darker or lighter skin tone that may be blotchy and uneven.

Achromia is usually caused by inherited recessive trait. There is no cure for albinism, and individuals should use a sunscreen at all times because they are much more likely to get sun damage and skin cancer.

Four out of the ten leading causes of death are: coronary heart disease, cancer, diabetes and cerebral vascular accidents are associated with unhealthy dietary patterns. Majority of people do not eat enough fruits or vegetables. It is true that poverty (especially in developing countries) is the main cause of unhealthy food habit. Those who can afford in place of required quantity of fruits or vegetables eat too much fatty foods.Elderly persons are at increased risk for poor nutrition due to the fact that they have multiple chronic illnesses. Some of them have tooth or mouth problems that interfere with their ability to eat. The elderly may be tacking multiple medications that can cause changes in their appetite. Negativity is also termed as pessimism. It is a way of approaching the world with pessimism. A person with negative attitude towards life assumes that whatever happens is somehow, will take away one’s well-being, peace of mind or life satisfaction. It is a serious barrier to positive ageing. Negativity creates false fear and impotence. Person starts thinking that nothing can be done to change the situation.

RIGIDITY : Barrier to Positive Ageing

“Mr. Srivastava,72 years of age, a retired bank manager, whose wife died 15 years back, lives alone in a big house. His two sons and a daughter live in different cities. They come to meet him at in turn, regularly. Mr. Srivastave fell ill and was diagnosed cancer by doctors. Doctors advised him to go to Mumbai for treatment. He was asked if he takes proper treatment at this stage his cancer may be cured. His children wanted to admit in a good cancer hospital in Mumbai, but he refused to be admitted in the hospital. He will not listen any body’s suggestions and does not want to leave his house. He had a typical view about his illness and treatment. Due to the stubborn avoidance of doctors by Mr. Srivastava his children were unable to force their father to regular medical checkups and proper medical care.”

This is an example of rigidity. Rigidity is a world view inconsistent with healthy adaptation. It is a major barrier to good mental health. Rigidity is a form of resistance to change, even when there is clear evidence that change will improve wellbeing or lead to healthier functioning. Rigidity can be problematic in coping. It limits one’s ability to see problem from different perspectives. Rigidity fosters repeating the same mistake again.

Retirement: Role Changes / Transitions

Retirement is perhaps one of the most common role changes faced by the ageing. This is an occurrence that either can be planned for and anticipated, may be sudden and unexpected related to illness or injury, or may present along some continuum between the two. Some people find a lack of purpose whereas others feel a new sense of personal freedom and time for activities postponed during busier years. Some people become busy in some kind of social work around them. Persons who retire unwillingly, may be at greater risk for unsuccessful adjustment—including alcoholism, depression, and suicide.

The major transitions after retirement are : Health transition, loss of spouse, role reversal etc. These factors influence independence in later life or life after retirement. Health , personality, state of mind and emotional, physical and spiritual support all have a place in the adjustments one makes to the ageing process.

Although self care and health promotion are indeed important in maintaining independence, ageing and accompanying health factors often make this a very difficult period of life. High functional ability is not absolutely necessary for high quality of life.

Ageing related protein can cause breast cancer


New research from Vanderbilt-Ingram Cancer Center have discovered that a most common form of breast cancer in older women called estrogen and progesterone receptors is caused by a protein disappointing off age-related cell damage.

David Gius one of the researchers told us that biological relationship between cancer and aging is one of the most established in cancer research. He throws light on protein, called sirtuins, which are combined with an extended lifespan seen in some species a calorically restricted diet.

The link between aging and cancer is one of the most established events in cancer. It appeared that some of these mice, sirtuins activity of an enzyme called MnSOD decreased. Moreover, agents that acetylation of this amino acid on target MnSOD may be useful as chemo preventive therapies in women at risk for these cancers and recurrence, he noted.

Advice of Delany Sisters On Maintaining Independence in Old Age

(Miss Sadie Delany, aged 106, and her sister, Dr. Bessie Delany, deceased at 104, had definite rules for living a long and healthy life. Both exercised every single day, whether they felt like it or not. Both always downed chopped garlic and cod liver oil at breakfast, and ate at least seven vegetables at lunchtime. The sisters also made a habit of living as stress-free a life as possible. They steadfastly refused to install a telephone, preferring more personal methods of communication. Other strategies for the sisters' serenity were published in the Delany Sisters' Book of Everyday Wisdom in 1994).

Bessie and Sadie, the Delany sisters are famous for remaining physically and mentally active in to their second century. After they became 101 and 103 years of age respectively, the book on their experiences was best-seller book, the television shows were very popular.
